Carbon Health Launches Primary Care-Led Metabolic Health Assessment Program

Carbon Health, a leading omnichannel healthcare provider, today announced the launch of its new Metabolic Health Assessment program, leveraging continuous glucose monitoring (CGM) technology to identify high-risk individuals for diabetes and guide them to appropriate care. Through seamless integration of hardware, software, and in-person and virtual care, Carbon Health is the first primary care provider to create a broadly accessible risk assessment program integrated with CGMs to identify and connect at-risk patients to primary care and endocrinologists.

Anyone, whether or not they have been to Carbon Health previously, is welcome to enroll in the program if they are clinically eligible based on a risk assessment modeled after the ADA prediabetes test. Patients who enroll in the program will be prescribed a CGM device to capture a two-week baseline of continuous glucose data. Over the course of the two weeks, patients will track in real-time their bodies’ responses to nutrition and lifestyle changes. Patients will then have a follow-up primary care visit with a Carbon Health provider to review their CGM data and develop a personalized care plan including lifestyle recommendations and, if appropriate, a referral to Carbon Health’s virtual endocrinologist-based program, Steady Health.

Carbon Health’s integration of CGM data with its omnichannel primary care model short-circuits the disease progression of prediabetes, which is often not visible for many years. Upwards of 85% of people with prediabetes are not aware they are at risk; identifying metabolic risk and treating diabetes is an otherwise complex process requiring multiple appointments and referrals. The standard screening practice for prediabetes or diabetes is to obtain a HbA1c and fasting plasma glucose measurement; however, relying on these two tests alone will often miss people with impaired glucose tolerance and isolated postprandial hyperglycemia. CGM’s promise in at-risk patient populations lies in the resolution of the continuous data to identify subtle changes that may indicate risks earlier than current clinical assessments can.

Currently, California residents between the ages of 18 and 65 whose healthcare is not covered by Medicare or other government programs are eligible to apply for the assessment. Carbon Health plans to expand eligibility to individuals nationwide in the coming months. The cost of the program for the patient is equivalent to the cost of a primary care visit.
