Synzi,
a leading virtual care company, has announced that Imperial
Home Health
(Imperial), a Medicare-certified home healthcare agency,
has selected its virtual care communication platform to pioneer the
delivery of better patient management, engagement and care in southern
California.

Imperial provides in-home clinical care to patients in the High Desert
community of California and was looking for a virtual technology
solution to improve patient experience and satisfaction.

Imperial selected Synzi’s virtual care platform for its comprehensive
video, email, text, and secure messaging capabilities. As a result,
Imperial will be able to better support referral sources, such as
hospitals and skilled nursing facilities, which are focused on driving
patient engagement to reduce readmissions and improve outcomes.

“We are committed to delivering high quality and cost-effective home
health care in response to our region’s needs and we were interested in
introducing technology to better engage patients, utilize staff
resources, and minimize rehospitalizations. With Synzi’s platform,
Imperial is well-positioned to provide our community with better care
and better outcomes,” said Eleazar Tayag, PT, DPT, MPH, GCS, COS-C,
Clinical Manager and Co-owner of Imperial Home Health.

Synzi’s award-winning communication platform leverages everyday devices
such as smartphones, tablets and laptops to help home health agencies
provide leading-edge, cost-contained, patient-centric care. Imperial’s
initial platform deployment is being executed in early July and will
enable Imperial’s staff to use multi-way video to conduct virtual visits
with patients and additional participants such as a family member, a
specialist, and/or a pharmacist.

“By implementing a virtual care solution, Imperial is differentiating
itself as an innovator in the competitive home care market in southern
California,” said Lee Horner, CEO of Synzi. “As the healthcare ecosystem
faces increased pressure to lower the cost of care and improve outcomes,
the Synzi platform gives Imperial the ability to strengthen patient
engagement and optimize staff productivity while helping reduce
unnecessary and costly readmissions.”

Imperial will also use the Synzi platform to schedule a series of
appointments with their patients and send appointment reminders via
phone, secure messaging, or email – depending on their patients’
preferred communication channels. With the platform, Imperial can also
program a series of patient-directed messages promoting medication
adherence, diet and lifestyle guidance, and general healthcare tips.
